Certificate of life

If you live outside the Netherlands you will receive the form 'Certificate of life' every year. Its purpose is to prove that you are still alive.

Why do we need this proof of life?
The Dutch registry offices automatically inform ABP about changes that occur in marital status, address, and death. Foreign registry offices do not inform ABP about these changes. Therefore it is very important that you notify ABP of these changes (eg in address) yourself. Filling in and returning the Certificate of Life indicates to us that you are still alive and therefore payments will continue to be made.

Please see to it that the form 'Certificate of life' is completed and validated by an official of the Netherlands Embassy or Consulate, by an official of the registry office, or by a solicitor. Be aware that if you do not return the completed form to us in time we have to stop payments. 

Do you receive AOW? If you do, the Sociale Verzekeringsbank will send the form to you instead.
